Create a new transparent document in the dimensions of the photo you want to use. Using the text tool, in all caps in black, type the name in something bold like Arial and increase the font size until it takes up the entire width of the photo. Using the marquee tool, draw a box big enough to fill up the the document up to where it just touches the edge of the text. Fill it with black. Merge the layers together.
