I'm not into the whole "the villain is always just misunderstood", BUT, if you're gonna write a good villain, write one that has a fully developed convincing backstory with enough sympathetic traits to make him human. Also there are times when he may genuinely be misunderstood by the hero, as we're finding out in our latest story. :)
They are just cheap imitations of real human beings. After growing up with an alcoholic narcissist father, and a narcissist sister, once is see someone is fake I'm gone. A few of them have been harder to see for what where because they were covert narcissists, I grew up with overt narcissists. Coverts leave you feeling the same way overts do, they are just sneakier about it.
