I really love this discipline chart. More detailed than smiley faces, the kids can go up or down with their clothespins in class and then it's a great way to communicate with the parents in their take home folder every day.
A Home Note Program to Support Positive Student Behavior and Outcomes
Our Special Day Class teacher asked me to create a simple communication log that can be filled out throughout the day and be sent home to parents to let them know how a student did.It is very simple, but editable (Word), so feel free to make changes as needed :)
Student Behavior Log: because I'll be an IS Teacher I think this will be a great tool to figure out when my students are acting up and try to solve the problem. Such as: If it's before lunch time every day then maybe they are just hungry.
Weekly Behavior LogDaily boxes for teacher to write notes in or put a stamp/sticker on. You can also have your students color in the box if you use a color system. At the bottom is a weekly report for the teacher to fill out.
